The personal data collected here allow the PSTQB - Portuguese Software Testing Qualifications Board to start the application process for ISTQB® Certification Examination.

With regard to the collection, processing and treatment of personal data:

1. A PSTQB - Associação Portuguesa de Testes de Software, rege-se pelo disposto no EU 2016/679, do Parlamento Europeu e do Conselho, de 27 de abril de 2016 (Regulamento Geral de Proteção de Dados (R.G.D.P.)) e, também, pela legislação e regulamentação aplicáveis em Portugal.
   
   
2. A PSTQB - Associação Portuguesa de Testes de Software, é a entidade responsável pelo tratamento dos dados pessoais constantes no presente formulário.
   
   
3. AUTORIZO, no âmbito do processo de Certificação requerido através do presente Formulário de Inscrição, a partilha dos dados pessoais com as seguintes Entidades parceiras.
   
   3.1 Plataforma LPLUS Gmbh: Nome completo, morada completa, código postal, cidade, país, data de nascimento, contacto telefónico, endereço de e-mail, nível de Exame a realizar, data e horário para a realização do Exame de Certificação;

   3.3. Plataforma Successful Candidate Register (S.C.R.) do ISTQB®: Nome completo, nível de Exame realizado, data da realização do Exame de Certificação e número de controlo do Certificado.

4. Ressalve-se que as finalidades aqui elencadas não serão as únicas, podendo outras derivar destas na medida do possível. Se existir uma nova finalidade independente das anteriores a PSTQB - Associação Portuguesa de Testes de Software informará, caso a caso, o titular no momento da recolha dos dados pessoais.
   
   
5. As part of the legal obligation to which PSTQB - Portuguese Software Testing Qualifications Board is subject, your personal data may be shared with Police, Judicial, Tax and/or Regulatory Entities.

1. Candidates must submit this registration form no later than 15 days before the intended date of the exam. Forms sent after this deadline may not be accepted;

2. The minimum number of Candidates per Exam is 5 Candidates, and the maximum number will be dictated by room conditions and invigilator;

3. The Requesting Entity or the Candidate must pay for the Examination(s) at least 5 days before the date of the Examination. Otherwise, the PSTQB may refuse to hold the Exam and/or release the Exam results until payment is made and confirmed;

4. Cancellations can be made up to 10 days before the exam date;

5. In case of cancellation after payment, the Candidate may take the Exam on a new date to be designated according to PSTQB availability;

6. Exam repeats are paid for at the same rate as the exam. In case of absence, it is not considered a repetition, but a rescheduling;

7. Whenever the Candidate is unable to attend the Examination, he/she must give notice no later than 5 working days prior to the date of the Examination.
When this is the case, he/she has the duty to justify his/her absence and the right to take the Examination on a new date to be designated by the PSTQB;

8. If the candidate does not notify within this period and is absent on the date of the exam, it will be considered a no-show, having 5 days to present the respective valid justification. If no justification is found, a new registration and payment will be required.
